PMOS: When Your Hormones and Diet Are in Sync

PMOS (formerly known as PCOS) is an endocrine disorder affecting 6–20% of women of reproductive age. It is associated with several metabolic disturbances, including insulin resistance, obesity, and dyslipidemia. These are also linked to an increased risk of type 2 diabetes and cardiovascular disease. One of its key features is insulin resistance, which contributes to hyperandrogenism. This can lead to symptoms such as acne and ovulatory dysfunction.
The exact causes of PMOS are not yet fully understood. Both endogenous and environmental factors are thought to contribute to its development.
PMOS is far more than just an "ovarian problem." It is a complex condition involving metabolism, hormones, and the body's overall balance. Let's take a closer look at how diet, insulin, and hormones are closely connected, and why taking control of your nutrition can make a real difference.
1. What Is Happening Inside Your Body?
Imagine your brain as the conductor and your ovaries as the orchestra. In PMOS, this communication becomes disrupted, and the conductor starts sending the orchestra overly intense and poorly timed signals.
- The main messenger, luteinizing hormone (LH), is often elevated and signals the ovaries to produce more male sex hormones, known as androgens. This can contribute to acne and excessive hair growth.
- Follicle-stimulating hormone (FSH) is responsible for supporting the development of the egg toward ovulation. In PCOS, however, FSH activity may be insufficient. The follicles, which contain the eggs, begin to develop but fail to fully mature. Instead of ovulating, they stop developing and can accumulate within the ovaries. As a result, the LH-to-FSH ratio is often altered, with LH levels sometimes more than 2.5 times higher than FSH.
- Estrogen levels can remain at levels typical of the early part of the menstrual cycle. The body does not receive the hormonal signal required for ovulation. At the same time, estrogen can continue to be produced from excess androgens in adipose tissue and directly within the ovaries.
2. The Hidden Problem: Insulin Resistance
There is another major player closely linked to PMOS: insulin. Up to 75% of women with the syndrome experience insulin resistance.
Your cells become less responsive to insulin, so the pancreas has to produce much more of it to keep blood glucose levels within the normal range. This persistent excess of insulin can fuel several processes associated with PMOS:
- It stimulates the ovaries to produce even more androgens.
- It interferes with normal follicle development and can contribute to anovulation.
- It reduces the liver's production of proteins that normally bind testosterone in the bloodstream. As a result, more free, biologically active testosterone circulates throughout the body, potentially worsening the hormonal imbalance.
3. Why Is Diet So Important?
Because PMOS increases the risk of complications such as type 2 diabetes, cardiovascular disease, metabolic syndrome, depression, and anxiety, early management and lifestyle factors play a major role.
Research suggests that an unhealthy diet, typically one high in calories and saturated fat and low in fiber, can significantly worsen clinical symptoms and metabolic disturbances. Studies such as Szczuko et al. (2017) have also reported associations between dietary patterns and impaired ovarian function.
The Gold Standard: The Mediterranean Diet
The Mediterranean diet is receiving increasing attention in the prevention and management of PMOS. Its key principles include:
- regular intake of healthy unsaturated fats
- plenty of high-quality protein and dietary fiber
- foods with a low glycemic index
- antioxidant-rich foods and essential vitamins
Researchers are also investigating specific nutrients that may influence insulin sensitivity, oxidative stress, and inflammation, including omega-3 fatty acids, vitamin D, magnesium, zinc, compounds such as myo-inositol, as well as various phytochemicals and polyphenols.
Regular physical activity is another important pillar. Ideally, exercise should be performed at least three times per week, combining aerobic and resistance training.

The good news is that every meal sends signals to your body, including your metabolic and hormonal systems.
